Home & Community Base Waiver
Services
Associated Community Services, Inc. (ACS) provides supported living with or without transportation.
Supported Living Services include 24-hour supports as wells periodic supports for up to 16 hours daily based on personal needs. Supported Living Service is provided by ACS in a home serving with one to three people. Supported Living is a blended service that covers habilitation, personal care, nursing, and other residential supports. Supported Living Services with Transportation ensure the provision of transportation services to gain access to Waiver and other community services and activities.
Companion Services are non-medical assistance and supervision to support a person’s identified goals, desires, and needs. Goals may be related to the person’s safety, promotion of independence, community integration, and/or retirement. ACS provides companion services to people also receiving Supported Living with ACS. Supports can be provided in a person’s home or in the community.
ACS provides Host Home Services which enable people to live in the community in a family-type setting and support them to achieve their personal goals, participation in community life and activities, maintain their health, and retain or improve skills that are important to them. This may include activities of daily living, money management, travel, recreation, cooking, shopping, use of community resources, community safety, and other adaptive skills.
In-Home Support Service provides people with the opportunity to continue to live in their own home or live in a family member’s home and receive supports. In-Home Support services are blended services that provide habilitation, personal care and other support services to the person in their home. Associated Community Services (ACS) supports the person to reside successfully in their home as their primary place of residence.
